- Couple’s Spa Massages: A completed medical history/intake form is required before scheduling your appointment.
- Advanced Medical Techniques: Services such as Manual Lymph Drainage and Myofascial Release (MFR) require a medical history form prior to booking.
- Oncology Massage: As a proud Recommended Community Provider for Memorial Sloan Kettering Cancer Center, oncology clients may also need to fill out forms before scheduling an appointment.
- Insurance Requirements: For clients using no-fault insurance or seeking reimbursement from providers like NYSHIP or Empire, a diagnosis and prescription from your healthcare provider are required in addition to the appropriate forms.
- Massage Spa Parties: Guests must complete their forms prior to the party date.
New York State Law:
NYS law requires that we maintain a record for each patient that accurately reflects the evaluation and treatment provided. These forms help us comply with this regulation while ensuring you receive personalized care.

Insurance Requirements
If you’re using insurance for your visit, here’s what you’ll need:
- A completed insurance form.
- A diagnosis and prescription from your healthcare provider.
Many insurance companies, including NYSHIP and Empire, reimburse clients for sessions. For auto accident clients, we accept no-fault insurance. Be sure to fill out the required forms prior to your appointment for a smooth billing process.
